- Order/Pay Online for the Test
- Select "In-Person Test" on check-out.
- Schedule Office Appointment on check-out/thank-you page.
- Come to our Office for the Test/blood draw.
- Get results electronically.
- This method works best if you are in the Greater Chicago area.
Order-To-Ship Test Kit:
- Order/Pay Online for the Test
- Select "Ship Test Kit Option" on check-out.
- Do your Test/draw and ship it back to us.
- Get results electronically.
- Free shipping on orders of $400 or more
- Orders $399 or below will incur a shipping fee of $65.00.
- Prepaid Return Label will be included in the Testing Kit.
